What is Breast Augmentation Surgery?

Breast Augmentation or breast enlargement surgery is the process of increasing the volume of the breast to the desired size, by using specially produced implant materials (silicone prostheses). Breast Augmentation Surgery is also performed to resolve sagging in the breast tissue, and provides the opportunity to achieve a firm and attractive result.
Breast Augmentation surgeries are one of the procedures which provide the most satisfying results within all of the cosmetic surgical procedures.

Why are Some Breasts Small?

Breast tissue may be smaller than desired depending on factors such as genetic disposition, unwanted side effects of medications, or volume loss after breastfeeding.
What is Considered to be an Attractive Breast?
Although the concept of "beautiful breasts" varies from society to society and personal taste, it is possible to talk about some general criteria. 

Let's start with the position of the breast tissue. For a person of average height and weight there should be a distance of 19-22 cm from the collarbone to to the nipple of the breast. However, this size may vary depending on such variables as anatomical structure, the size of the breast tissue and the size of the patient. For example, if you are 190 cm tall, then the 19-22cm distance is too short.

Ideal breast size also differs from society to society and from person to person. Of course, it is possible to talk about the ideal size for a person of average height which the majority of people find to be ideal. The distance from the collarbone to the nipple is 19-22 cm, the distance between the nipple and the lower border of the breast is 8-9 cm and the mean diameter of the nipple is 4 cm. These measurements are perceived as attractive by the majority of people.

Which Type of Implants are Used in Breast Augmentation Procedures?

There are different kinds of implants that have been developed for Breast Augmentation and different techniques are applied according to the type of implant. The patient's expectations, the condition of the current breast tissue and the anatomical structure of the patient, play a key role in the selection of the method and the type of implant to be used.
The implants used world-wide are divided into two groups, those filled with silicone and those filled with saline. Implants filled with soy were used for a period of time but their use was not widespread. Over the years, breast augmentation surgeries show that the most ideal implants are silicone gel-filled implants.

Why are Saline-filled Implants Not Preferred?

Saline-filled implants have some disadvantages; such as the water sound the implant produces and the inconsistency of saline with that of the breast tissue. In addition, the new generation silicone gel-filled implants have very high endurance, and do not cause undesirable conditions such as perforation or tearing that were previously encountered by patients. High-quality, FDA approved silicone implants have lifetime guarantees for such conditions.

Why are Silicone Implants Preferred?

New generation silicone filled implants, are now very resistant to punctures and abrasion. Even if a puncture or tearing occurs, the gel inside the casing is made from non-flowing silicone (cohesive gel). Therefore, nowadays silicone filled implants are used safely and they can maintain their breast shape where they are placed for life without the necessity of replacing them at certain times like the ones used previously.
In silicone implant applications, there are no adverse consequences such the inconsistency of the material of saline implants with that of the normal breast tissue and difficulties with the noise created by the saline filling.

How is the Size of the Implant to be Used Decided?

Pre-operative planning is very important in breast augmentation surgery. Before the surgery, your expectations for size and the size deemed anatomically appropriate are considered together. Theoretically, the size of the breast should be consistent with the patient's height, chest diameter, and hip diameter. Planning will be carried out according to the evaluation that you and your doctor make together.

What is the Anatomic Area in Which the Breast Implant is Placed?

There are alternatives for the area in which the breast implant is placed in the breast augmentation procedure. Beneath the breast tissue is the muscle pectoral muscle that extends from the shoulder to the lower border of the ribs. According to the volume of the existing breast tissue, the breast implants are placed under or over this muscle.
Implants can also be placed in a procedure that is known as "dual plan". That is, that part of the implant can be placed above the muscle while the other part can be placed below the muscle. The appropriate technique will be decided according to an evaluation of the patient's breast tissue, the anatomy of the body, and the planned enlargement.

How is the Breast Augmentation Procedure Performed and Where are the Scars Placed?

An incision needs to be made in order to place the implant under the breast tissue. The incision may be made in one of three places,
    •  Around the areola or
    •  In the armpit area or
    •  In the crease of the breast

When the implant is inserted by making an incision around part of the areola, the scarring blends with the colour already present in the areola.
When entered through the armpit, the incision will be hidden in this area.
When the breast crease is used, the incision in this region disappears over time as it will correspond to the skin fold formed by the breast tissue.
After the required area is provided by entering from these three regions, the implant can be inserted. When the implant is inserted through the crease of the breast there is an incision made of approximately 3 to 4 centimeters. Because this is a natural crease found in the body, it is not easily visible. Through post-operative laser application, we are capable of making the trace further unnoticeable. For these reasons, the most appropriate approach is to perform the surgery through the crease of the breast.
In addition to this information, saline-filled implants can be inserted through the the belly button. This means that the incision and resulting scar remains in the belly button area. However, this method is not possible for silicone filled implants and saline-filled implants are not the implants that are recommended in breast augmentation.
The entry point to be used is always determined with the patient and the doctor together, and the patient is fully informed of the process and details of the surgery.

Can the Resulting Scars of the Breast Augmentation Surgery be Minimised through Laser Treatment?

It is best to perform the operation by entering through the breast fold so that the incision is not visible. In this way, the patient does not have to worry about scarring.
Apart from this, wound healing and therefore the remaining scars differs from person to person. In some people, the genetic disposition towards wound healing may be greater than in others. In cases of poor healing, it is possible to minimize the formation of scar tissue through the use of Fractional Laser treatment.
As with all cosmetic surgery procedures, the important thing is to ensure that the incision is placed in an area that is not directly visible and that the incision is made at the minimum possible length.

How Can I Know Which Implant and Which Technique is Best for Me?

The type of breast implant to be used and the region that the implant will be placed should be identified according to the expectations of the patient coupled with the patient's individual situation. For this reason, decisions should not be made according to the comments of others who are not qualified and experienced in the field of aesthetic surgery.
The best way to move forward is to assess the expectations of the patient, and for the plastic surgeon to determine which methods will be most suitable for the best outcome possible.

What Can I Expect at My First Doctor's Consultation?

Your Plastic Surgeon will listen to your expectations for the surgery during your first consultation. Your doctor will perform an evaluation of your current situation and then share the details of the procedure to be performed in order to achieve the desired results.

What Kind of Anesthesia is Used in Breast Augmentation Surgery?

Mostly general anesthesia is preferred. 

How Long on Average Does Breast Augmentation Surgery Take?

The surgery lasts approximately 1 to 1.5 hours.

What Should I be Aware of Before the Procedure?

    •  Smoking is a risk factor for any surgical procedure. It is recommended that you do not smoke for a month before and after the Breast Augmentation procedure, especially because it can cause discomfort and additional complications in wound healing.
    •  It is recommended that you do not take aspirin or medications that contain aspirin 2 weeks prior to surgery.
    •  It is recommended you do not take alcohol the night before the operation so that dehydration does not occur.
    •  You should not eat or drink anything 8 hours prior to the surgery.
    •  Following the surgery, we recommend that you have comfortable clothing with you for your stay in the hospital (tracksuit, nightgown with buttons down the front etc.).

What Can I Expect Following the Procedure?

You are taken back to your room a short time after the procedure is completed. There is usually no pain, or very little pain following the procedure. In the case of pain, analgesic medication can easily provide relief.
After the surgery, you can return everyday activities and work after 4 to 5 days provided you don't make sudden or large scale movements. It is normal to experience some pain such as you would in your muscles after training or a work-out, for a period of up to 10 days or so. The sutures used will be absorbed by the body and disappear in about a week.
About a week after the surgery, once your incisions have had the chance to heal, your doctor will show you some special massages to apply to your breasts on a regular basis. These massages will help to provide your breasts with the best possible shape and form.
