What is a Dermal Filler?

Dermal fillers are used to raise the area it is injected into. They can be used to smooth lines and wrinkles on the face including lines on the forehead, around the nose, mouth and lips. The dermal fillers smooth the lines and wrinkles and reduces their depth.
At the same time, dermal filling is the procedure of choice to add fullness or volume to areas such as the lips, cheeks, cheekbones and chin.
Among the most commonly used and safest fillers, are temporary fillers which contain hyaluronic acid, a substance which is already present in the body and is accepted without reaction. Dermal fillers containing Calcium Hydroxyapatite (CAHA) also provide successful results.
While hyaluronic acid is preferred for areas containing soft tissue such as the lips, other fillers are preferred to provide volume or fullness on and around bone structures such as the jawline and cheekbones. As with any cosmetic intervention, the use of FDA approved products in dermal filling procedures is an important consideration. At Istanbul Surgical Arts, we do not use any dermal fillers that do not have FDA approval.

Is a Permanent Result Possible through Dermal Filling?

The use of permanent dermal fillers in cosmetic procedures have been discontinued due to the negative effects experienced by patients in the long run. We acknowledge and agree with this approach as we care about the health and long-term satisfaction of our patients.

Are Dermal Filling Procedures Safe?

At Istanbul Surgical Arts, we are able to confidently assure all our patients that we use FDA approved devices and materials in all our procedures, which have no harmful long term effects for our patients. Thus, we ensure that our patients have access to reliable brands and safe products through the services we provide.
Amongst temporary, dermal fillers, the most commonly used and safest, are those containing hyaluronic acid. These fillers do not provoke a foreign substance reaction in the body, as hyaluronic acid is already present in our systems. Hyaluronic acid is a substance found in many tissues physiologically in the human body, so the body does not show a reaction to this substance.
Long before they were used in the field of cosmetic procedures, fillers have been used in many different medical fields for a variety of treatment purposes, especially orthopedics. This is a good indication that temporary fillers can be safely applied in current procedures.

How Long do the Effects Last?

The effects last 8-12 months, during this time the filling material is broken down by the body and removed from the area.

Who is a Suitable Candidate for Dermal Fillers?

    •  If you are pregnant or think you may be, or if you are breastfeeding, the injection of dermal fillers is not recommended.
    •  If you have a known protein allergy, dermal fillers are not recommended.
    •  If you have an active acne problem, your doctor will alert you to the situation. In such cases, the procedure will not be performed, or alternatively a number of additional treatments will be performed before the dermal filler procedure is carried out.
    •  Your specialist may detect a different reason as to why the procedure cannot be performed as a result of the examination that is carried out.
    •  If you have any medications that you use regularly, or have any other illnesses or medical conditions, please tell your doctor.

What Should I be Careful About When Having the Dermal Filler Procedure?

Every procedure, although minor, have their share of risks.
    •  You should evaluate the risks of dermal fillers together with; alternative procedures available to you, the method that will be used to apply the dermal filler, your doctor's recommendations, and the general advantages and disadvantages of the procedure. Don't forget to ask your doctor any questions that you may have.
    •  The dermal filler procedure is a safe method that has been used for many years. However, the application of unsuitable, undefined ingredients in filler materials often results in undesirable and irreversible consequences that are frequently reported on the news. Please ensure that the filler material used is FDA approved and a reliable brand.
    •  When dermal fillers are applied by those who are not specialists, swelling in the application area, permanent color changes in the skin, granulomas, long term swelling and allergic reactions can occur. Ensure that your doctor is a Plastic Surgery Specialist or a Dermatology Specialist.

Are There any Undesirable Effects that Can Occur After Dermal Filling?

Very rarely temporary effects can occur after dermal filling;
    •  Occasionally bruising and swelling (edema) occurs in the application area. These effects disappear in a few days.
    •  Very rarely, a temporary allergic reaction may occur to the filler material. Additional treatment and procedures may need to be applied in these situations.
    •  Rarely, asymmetry can occur in the face or application area caused by swelling. Within a few days, the swelling will subside. If the asymmetry remains after the swelling has subsided, your doctor will address this situation through the application of additional filler material.
    •  Very rarely the filler can cause a reaction under the skin. In this case, there may be pinky-redness on the skin, and hardness in the application area may occur. Those who carry the herpes simplex infection can experience cold sores. If you do experience cold sores, inform your doctor so they can provide you with additional treatment so this situation is prevented.

What are the Alternative Methods of Dermal Filling?

Botulinum Toxin treatment can be applied on the forehead, between the eyebrows and on the lines around the lips. Volume can be increased through the use of a variety of implants, or surgical interventions such as a facelift are alternatives to dermal filling.

Which Areas are Suitable for Dermal Filling?

Some of us have thin lips and imagine ourselves with fuller ones. As some of us mature, we notice that our lips begin to get thinner and vertical lip lines start to appear.

For this reason, the most popular area in which dermal fillers are applied worldwide is the lip region. However, in the beginning some patients shy away from this procedure, as we often see exaggerated and ungraceful dermal filler application results. In fact dermal lip filling procedures in the hands of a skilled specialist, applied using the most appropriate method, can provide a natural and attractive result that does not draw the disturbing gaze of others.

The technique to be chosen will be based on the needs and the expectations of each patient. The filler material to be used should be FDA approved, contain hyaluronic acid and be specifically produced for the lip region.

The purpose of plumping the lip is not to plump the lips beyond normal proportions but to provide volume and beauty by turning the lip out slightly to a reasonable level. Increasing the visibility of the pink mucosa part is a way of doing this. The expected result is not to draw attention to an abnormally thick lip, but to provide an eye-catching and graceful look.

In addition to this, a more natural and beautiful result can be achieved by adding height and volume to the two lines (filters) running parallel to each other between the lip and the nose.

When FDA approved material is used in the lip filling procedure, the result lasts about 8-12 months on average. As the filler material is slowly broken down by the body, the result will gradually decrease over time. However, a repeat of the procedure is not recommended before a year has passed.

Marionette Lines

If the lines that extend downward from our lip corners are deep, they are called marionette lines. If these lines are deeper than usual, they create a sad or unhappy expression that is noticed even in photographs. Additionally, the marionette lines have an impact on the lines that descend from the nose down to the rim of the lips, making them appear deeper and longer than they actually are. This happens because visually they appear as a continuation of the marionette lines.

It is possible to reduce marionette lines with a very short session of dermal filling. The sad expression created by these lines will be replaced by a more positive one and though the area from the nose to the corners of the mouth are not included in the procedure, a significant softening and diminished appearance is created here also.

Again, the filler to be used must be an FDA approved hyaluronic acid product. The effect will last 8-12 months.


The height and volume of the cheekbones is key for a beautiful face. Supporting cheekbones that are low or inadequate in volume through the use of filler can create a positive result, without sacrificing a natural look.

Raised and projected cheekbones reveal beautiful facial features. In addition, softening also occurs in the lines between the nose and the corners of the mouth due to the small lifting effect the increased volume creates in the middle-face region. This inevitably leads to a desirable and positive result reflected in the entire face.

Conducting dermal filling in the cheekbone area may not be appropriate for every patient. The suitability of the procedure will be determined through consultation with a plastic surgeon. Again, as in all cosmetic procedures, despite wishing for a positive result, it is possible to achieve an effect that is unnatural, exaggerated and disturbing. Cheekbone dermal filling can provide natural and satisfying results if the appropriate quantity is used, carried out with the right application technique, taking into consideration the individual needs of the patient.

The filler to be used must also be FDA approved hyaluronic acid or Calcium Hydroxylapatite (CAHA). The result lasts 8-12 months.


The chin is one of the key components of a beautiful face structure. The protruding nature of the chin exists only in humans and makes a positive contribution to the appearance of the face both in men and women. Through the use of FDA approved products containing hyaluronic acid or Calcium Hydroxyapatite, dermal filling applied at the tip of the chin enables a very attractive and natural effect on the overall appearance of the face.

Dermal filler applications, provide satisfying and attractive results especially in the treatment of mild to moderate chin imperfections. If the chin is receded to a more advanced degree, then a chin implant may be a more appropriate way to move forward to produce a more effective result.

Under Eye Light Filler

Under eye filling, otherwise known in the media as light filling, is the injection of fillers containing hyaluronic acid. The main purpose of the under eye filler is to eliminate the dark circles under the eye area and to reduce the size of the pit in the corner of the eye often referred to as the "tear pit", which has increased in depth in some patients.

Because the skin under the eye is very thin, it is perhaps the riskiest procedure in dermal fillings. There is a high risk of complications, and there is also a high risk of long-term problems that can occur. Due to the thin skin in this area, even the slightest irregularity can create a negative outcome and be noticed by others. Frequent repetition of under eye filling can lead to long-term and difficult-to-fix problems due to the anatomical characteristics in the under eye region.

Due to the long-term problems that can occur with this procedure, most plastic surgeon specialists prefer to use autologous fat transfer (using the patient's own fat during the filling procedure) and recommend this to their patients. However, this method is a more extensive procedure than the application of hyaluronic acid containing fillers. First, a small amount of fat is taken from the patient using liposuction. In sterile conditions, the patient's fat, without the addition of any additives, is injected using suitable needles, the same way as the dermal filler procedure.

There is a natural and positive expression provided through the procedure, caused by the diminished appearance of the tear pit, and the distinctly lighter colour created in the under eye area.

The most appropriate procedure, under eye filing procedure or the use of fat transfer to the under eye area, will be determined by your plastic surgeon following an examination.

Fillers for Lines and Wrinkles

As skin matures, it loses its elasticity and moisture. This is also the reason for the parentheses (whistle lines) created around the nose and lips. Serums and particularly moisturizing creams are products that you will find useful to slow down this process.

However, correctly applied fillers ensure that volume is added to lines and curves to create a more natural effect, and will provide unparalleled satisfaction.
