The muscles around our eyes that help to form our expressions contract when we laugh or smile, get angry or sad, cry or are surprised. These muscles affect our skin, and create wrinkles in the surrounding area. As the same expressions are repeated over many years, the wrinkles created in the skin become a permanent part of our features. Simply put, Botulinum Toxin shows its effect by weakening the muscles and the surrounding area where the injection is made.

What is Botulinum Toxin That is Used for Wrinkle Treatment?

Even though Botulinum Toxin is popular known to be snake venom, this information is incorrect. The ingredient used during Botulinum Toxin application is actually a bacteria called "Clostridium botulinum" which releases a protein "Botulinum toxin". The Botulinum Toxin breaks down the electrical communication between the nerves and the muscles and therefore reduces the muscle function in the areas it is applied to. When muscle function is lost, the muscle cannot contract, this causes the wrinkles in the skin to reduce or disappear completely. In this period the skin has the opportunity to rejuvenate itself, and even when the effect of the Botulinum Toxin is gone, there are less wrinkles than previously before.
Botulinum Toxin can be combined with fillers and provide volume to the areas in which they are applied. However, the effect of Botulinum Toxin is created by its action on the muscles.

How is Botulinum Toxin Applied for Wrinkle Treatment?

The skin is cleaned and an anesthetic topical cream is applied, which takes effect after a short period of time. The patient is then propped back slightly. The doctor will inject the Botulinum Toxin using very thin needles into different muscles at the appropriate angle and locations. During the application you may feel small pricks as the doctor injects the Botulinum Toxin, you may also feel a slight burning sensation. 2 to 10 units of Botulinum Toxin is used during the application, the amount to be used will be determined by the doctor taking into consideration the patient's wrinkles, muscle size, and muscle function. For this reason, the dosage will be different for each patient.

How Long Does the Wrinkle Treatment Take?

The whole process takes approximately 10 to 15 minutes. The wrinkle treatment session is a short but effective cosmetic procedure. It's possible to conduct the session easily and effectively in our medical offices whenever you wish.

When Does Botulinum Toxin Take Effect and How Long Does it Take?

The effects of the Botulinum Toxin treatment will begin to take effect in 2 to 3 days, and its maximum effect is seen within 3 to 7 days. Reapplication is recommended in approximately 4 months. The effects of Botulinum Toxin are longer lasting with regular re-applications.

What Follow-up Care is Necessary After the Wrinkle Treatment?

The follow-up care is different for each person according to the patient's individual needs and previous wrinkle treatment experiences. Generally however, it is recommended that for the 24 hours following the application the patient minimises the use of their facial expressions. The injected areas should not be massaged in the two days following the application, care should be exercised especially when removing make-up, the affected areas should be handled gently. If care is not exercised, the medication could spread to other areas and not show the expected effect. The face can be washed and make-up can be applied 2 hours following the treatment. Activities such as pilates, step and other exercises, including any heavy lifting should be avoided for 24 hours, solariums and saunas should also be not be visited.
The application will take full effect within 4 to 7 days. At the end of this period your doctor will touch-up the application if he sees fit, at no additional charge.

What is the Ideal Age for Wrinkle Treatment?

When Botulinum Toxin is applied at an early age, it helps to prevent early ageing (wrinkling), and provides a smooth and radiant appearance to the skin. If Botulinum Toxin treatment is started at a later age where wrinkle formation has already begun, wrinkling is reduced and its further development prevented.

Why is an Individualized Approach to Wrinkle Treatment Important?

Wrinkle formation is affected by a variety of factors including; the size of the muscles, their shape and function, how facial expressions are used, previous exposure to the sun, the structure of the skin, the age, and sex of the patient. Therefore, the Botulinum Toxin dosage and injection sites will be different for each patient.
The doctor to apply the wrinkle treatment should have the expertise to be able to evaluate all these parameters and offer a treatment that is individual to the patient. Many of us see the fake, mask-like expressions that result due to poorly applied Botulinum Toxin treatments. These artificial expressions that we observe, come up when an individualized approach mentioned above is not taken into consideration. 

Is it Possible to Achieve a Natural Effect with Wrinkle Treatment?

If your facial characteristics and needs are evaluated correctly, Botulinum Toxin application can be a very effective and practical way to stop time. However, if wrinkle treatment is not designed according to the individual's needs the face can appear frozen, surprised or angry.
A patient with weaker muscles who has fewer wrinkles and another patient who has stronger muscles and deeper wrinkles should not receive the same dosage or application. If the same application is made, then the result will be unnatural, the facial expressions stunted, with a surprised or angry look resulting. That is why a standard application dosage cannot be applied.
With a personalised approach to dosage, and the correct technique and application in the right areas, a desirable result is possible. A radiant, wrinkle-free and natural face is achievable.

Does Botulinum Toxin Have any Negative Effects?

Botulinum Toxin has been used in the area of cosmetics for 15 to 20 years. It has been used for the treatment of muscular illnesses for much longer. Throughout its use, there hasn't been any identification of any negative systemic side effects. Botulinum Toxin is widely used throughout the world, and is also approved by the FDA. In the 2016 year alone, there were 6.5 million Botulinum Toxin treatments just in the USA. Clinical studies conducted over many years have shown that there are no short or long term ill side effects caused by the use of Botulinum Toxin. Most critically, the brand of the Botulinum Toxin to be used should be FDA approved.

Can Botulinum Toxin Application Cause an Allergic Reaction?

Botulinum Toxin carries the same allergic risk as any other medication. There is no need for a sensitivity test. Allergic reactions are rarely experienced and there are different treatments available should this occur.

What are Some of the Unwanted Side-effects of Botulinum Toxin Treatments?

Rarely, there may be redness or bruising experienced at the site of the injections. The redness will subside within a few hours, and the bruising within a few days. You may experience a headache, that will be relieved within a short time.
In rare instances, swelling may occur on the eyelids, however this will pass quickly. Very rarely, swelling on the eyelids (either one, or both eyelids) may occur, and last up to one week.
The complication that can occur with Botulinum Toxin application is a drooping eyelid. This can occur for two reasons, one is incorrectly applied injections (too close to the eye area, or rules for Botulinum Toxin treatment not being followed) or if the injected areas are rubbed by the patient and treated too vigorously despite contrary instructions given to the patient in this regard. These negative consequences are not experienced when the wrinkle treatment is carried out by an experienced plastic surgeon or a dermatologist.
Some patients may develop antibodies to Botulinum Toxin; this situation occurs when the patient has Botulinum Toxin applications too frequently. This does not pose a danger to the patient because the body easily breaks down the Botulinum Toxin.

When is Botulinum Toxin Application Not Recommended?

Botulinum Toxin treatment is not recommended for those who are or may be pregnant, breastfeeding mothers or anyone who may have any kind of muscular illness. You should share any medical conditions and / or medications that you use or with your plastic surgeon before the procedure. Your doctor will provide you with information regarding your candidacy for Botulinum Toxin treatment.

What are the Alternatives to Botulinum Toxin Treatment?

Alternatives that can be considered for Botulinum Toxin, are those procedures that address the treatment of wrinkles. After the patient is evaluated the doctor may recommend a number of different methods to address the ageing effect of wrinkles. If determined appropriate one approach may be the use of fillers to fill in deep wrinkles. Other effective alternatives are laser applications, the use of radio-frequency energy or ultrasound energy such as Ulthera.
However based on long term clinical studies, Botulinum Toxin treatment is the gold standard approach to crow's feet around the eyes, lines on the forehead and in between the eyebrows. In other words, Botulinum Toxin treatment is the easiest and most effective method.
