What is Rhinoplasty (a nose job)?

A rhinoplasty (nose job) is a surgery to correct and reconstruct the form of the nose to make it aesthetically pleasing. The aim is to produce an attractive nose contour that is in harmony with other facial features.

What is the Sign of a Successful Rhinoplasty?

As with any cosmetic surgery procedure, the intended result should be natural. After you have healed, and the swelling has subsided, you will know that your rhinoplasty surgery has been a success when you meet someone you haven't seen for a long time who says, "there is something different about you, you look good, did you get a haircut, did you lose some weight?".

How do I Choose the Right Plastic Surgeon?

The result trying to be achieved through rhinoplasty is very important. In fact, just focusing on the nose is usually the main fault of plastic surgeons who specialise in rhinoplasty. This is because rhinoplasty is not just about achieving an attractive nose, it is about  bringing about symmetry and balance to the face.

Aesthetic evaluation is an area of expertise. For this reason, it is important to choose a plastic surgeon whose area of expertise is rhinoplasty along with a strong aesthetic understanding of the anatomy in all areas of the face including; the periocular, jaw, cheekbones, cheeks, forehead, ears, etc. In this way, all the components of the face will be evaluated holistically, all alternatives will be considered, and the best intervention will be decided on. 

Why is a Combined Approach in Facial Surgery (Prophiloplasty) Important?

Rhinoplasty is generally influenced by all facial features.

The nose is the main component that makes up your profile. However, it would be a mistake to evaluate the profile taking into consideration the nose but not evaluating the other components of the face such as the chin, and cheekbones, which affect the profile also. 

Surgery can sometimes be planned only for retouching interventions to the nose, jawline and cheekbones; in the hands of a skilled plastic surgeon, very good results can be achieved. In some patients, the retouching can be focussed on the jawline and cheekbones alone, without any changes made to the nose. This combined approach is called prophiloplasty, not rhinoplasty. The approach of obtaining an attractive face, not just an attractive nose, is the current approach used by professionals in the field, the approach of evaluating the nose alone is a thing of the past.

However, this does not apply to every patient. Most patients do not have any problems with the other components of their face, the majority of patients who come to see a plastic surgeon are unhappy with the shape and size of their nose. In this case, plans are made to align the nose with the other components of the face. A plan for surgery is made based on the expectations of the patient, considering what is physically possible and the plastic surgeons recommendation and evaluation. 

What Can I Expect at My First Doctor's Consultation?

Your doctor's consultation will begin with your plastic surgeon listening your complaints and expectations. Your plastic surgery will listen to you carefully. This process is important in order for your doctor to have a clear understanding of your expectations. The doctor will examine your nose in detail including; the bone structure of your nose, the cartilage, the nasal cartilages at the tip of your nose and their associated configuration, and the septum which separates the nostrils 

The basic principle is to evaluate the entire face of the patient, in particular the  balance and harmony of the cheekbones, the chin and the nose. This is a very important point because the the way that the nose appears actually depends on the state of these two regions. For example, if the jaw is set back, it causes the nose to appear bigger than it actually is.  In such a case, an intervention made to the nose only would be both incorrect and insufficient.

What is Osteotomy?

The question frequently asked by patients and based on false information is, "Do you perform your surgery with or without osteotomy?" In other words, "Do you shape the bone during surgery by breaking it or by shaving it down?"

The fracture of the nasal bones during a rhinoplasty, called osteotomy, is decided on a case by case basis, and upon the need of the patient.

When is Osteotomy Performed?

The nose is unique to each individual and therefore each person's surgery will be different. The fracture of the nasal bones, called osteotomy, is decided on a case by case basis, and upon the need of the patient. It is not possible to correct a nose, which is wide or has a large arch on the roof, without fracturing it.

If the width of the nose is not a problem, or if the hump on the roof of the nose is small, then the rhinoplasty can be performed and the nose remodelled by shaving rather than fracturing the bones. In this case, patients will not need to have plaster on their nose after the surgery. 

Is it True That the Swelling, Bruising and Pain is Increased with Osteotomy?

If the patient has the above-mentioned conditions, then the bones must be fractured before they can be corrected. In the early days of osteotomy practice, patients were stuck with swollen eyes, bruising and a painful healing process. 

Today, if osteotomy is performed, there are no serious consequences as there were previously. New methods have been adopted in the osteotomy of nasal bones, the fracture is made using ultrasound, vibration and similar energies. The use of these methods has removed the fear surrounding the the pain that will be experienced following the surgery. 

The anesthesia and equipment used during the operation and new generation products and applications to be used after the operation also eliminate the undesirable results of bruising and swelling in the recovery period. Newly developed medicines, sprays, and gels, as well as ice treatment and other advanced techniques to be applied after the operation will provide comfort and relief.

The most important point to bear in mind here is to choose the right plastic surgeon specialist. In this way, the method to be used in the operation will be specified in a personalized manner, and successful results will be obtained in which the swelling, bruise and pain are minimized.

What is Rhinoplasty Without Osteotomy?

We have seen the rise of the abuse of rhinoplasty for commercial purposes through the use of lighter interventions. This has been done using the method of the shaving of the nasal bone which can provide ideal results, with the absence of swelling and bruising.  We have seen some strategic marketing and the use of jargon such as “"Rhinoplasty without Osteotomy"”. This type of marketing jargon causes misunderstanding for patients and can cause the patient to make requests for their surgery which are not appropriate to their personal needs. 
The choice of osteotomy must and will be made by the plastic surgeon, it cannot be made based on the patient'’s wishes or desires.  Osteotomy may or may not be necessary for the patient. The approach for rhinoplasty for each nose is different. The needs of each patient is different. 

What is the Difference Between an Open and a Closed Rhinoplasty?

Another thing to know about rhinoplasty is related to the techniques to be applied during the surgery. Basically, two methods are used in rhinoplasty: open or closed rhinoplasty.

In closed rhinoplasty, the surgery is performed through the nostrils. The incisions are inside the nose and there are none placed outside the nose. In open rhinoplasty, the front part of the nose is opened up by making an incision in the front section of the nose, the rhinoplasty is carried out through the opening. 

The decision to conduct open or closed surgery will be made according to the patient's situation and needs. At this point, it is important that the plastic surgeon who will perform the surgery is skilled in both methods. For the best results, you should find a plastic surgeon who is an expert in both methods. A physician who is a skilled practitioner in both open and closed rhinoplasty methods will be able to easily discern the best method to use in your situation. 

What Kind of Anesthesia is Used in Rhinoplasty?

Rhinoplasty is performed under general anesthesia.

How Long on Average Does the Rhinoplasty Procedure Take?

If taking into consideration all the facial features of the patient, the surgery will take 1 to 2 hours on average.

What Should I be Aware of Before Rhinoplasty Procedure?

    •  Smoking is a risk factor for any surgical procedure. It is recommended that you do not smoke for a month before and after the procedure, especially because it can cause discomfort and additional complications in wound healing.
    •  It is recommended that you do not take aspirin or medications that contain aspirin 2 weeks prior to surgery.
    •  It is recommended you do not take alcohol the night before the operation so that dehydration does not occur.
    •  You should not eat or drink anything 8 hours prior to the surgery.
    •  Following the surgery, we recommend that you have comfortable clothing with you for your stay in the hospital (tracksuit, pyjamas with buttons down the front etc).

What Can I Expect Following the Rhinoplasty Procedure?

You will not feel any pain once you come out of the surgery. If you do experience a slight burning sensation, it will disappear within 2 to 3 hours. After the surgery, you should only drink fluids for about 2 to 3 hours. After your blood pressure is measured, you can start to walk and start to eat normally. You will need to stay in the hospital overnight following the surgery. 

On the first day there is usually no bruising or swelling in the operated area. Swelling and bruising can begin within 10 to 16 hours of the surgery. To minimise its effect, we recommend the regular use of ice compression as a simple and effective solution.

After evaluating your general health condition and your nose you will be discharged the day following the surgery. Your nasal packing will usually be taken out by your doctor on the second day. 

If osteotomy has been performed, there is a splint (usually "plaster") used on the nose for up to 1 week after the surgery. The Splint application is needed to support and fix the reshaped bones in their new positions.

If osteotomy was not applied and the nose was shaped through shaving, the splint is used for a few days to prevent swelling.  Once the splint is removed approximately a week after surgery, the positive changes will be observable immediately. However, due to surgery-related swelling, the nose will not be in its final form. Most of the swelling,  called edema, will disappear within the first month and will no longer be noticeable. Of course, as with all other surgical procedures, it will take 6 months to 1 year for all the swelling to subside. This does not mean that the nose will appear unattractive and swollen all the time; as time goes by, the small details will become more fine-tuned and the shape will become better with every passing day.  

Is it Possible to Have Rhinoplasty Without Nasal Packing?

Rhinoplasty without nasal packing is possible for both open and closed surgery methods. Nasal packing is used to provide comfort to the patient following surgery as it absorbs small amounts of leakage from the nose. If nasal packing is not used a sponge or gauze dressing is placed at the end of the nose to catch any leakage. The dressing / sponge will be replaced every 10-15 minutes The leaking ends within the first 10 hours of the surgery.

The septum separates the nose into two and forms the wall between the nostrils. The use of nasal packaging is a must in rhinoplasty surgery where septum deviation is corrected, as the septum needs to be supported after the surgery. In order to achieve a permanent and desirable shape for the septum, the doctor will place nasal packaging into the nose while the patient is still under general anesthesia. 

Different types of nasal packaging are available. Silicone bumpers and extras are the most widely used types. Istanbul Surgical Arts as aesthetic nose surgery; we support the use of extra-padded bumpers that are soft, absorbent, highly resilient, and softened with the ointment so that they can be taken comfortably. The septum choice is made by a specialist physician when there is an intervention in the septum.

There are different kinds of nasal packaging that can be used. Istanbul Surgical Arts uses soft, highly absorbent, easily pliable nasal packing which is moistened and softened with ointment. The nasal packaging used is comfortably removed from the nose. In surgeries where the septum is corrected, the choice of nasal packaging may change, and the decision will be made by your doctor.

Is the Nasal Packaging Uncomfortable?

In rhinoplasty surgery, nasal packing is sometimes a concern for patients. The discomfort experienced while removing the long nasal packaging used in any kind of nasal surgery in the past is the main cause of this concern. Unless surgery has been conducted to improve the patient's breathing ability, only very short nasal packing is used. In order for the packing not to stick to the inside of the nose, a special ointment is used before it is positioned. Therefore, there is no pain experienced while the nasal packing is being removed. Many patients express their amazement at how easy and trouble-free it was to have the nasal packing removed.

Is It Possible to Improve my Breathing During the Rhinoplasty Surgery?

Rhinoplasty can be done to improve the cosmetic appearance of the face as well as to correct an important and basic life function such as breathing. If the patient has difficulty breathing, it is possible to solve this problem during the surgery if the doctor'’s evaluation and examination reveals that the difficulty is caused by the nose.

Can I Have Nasal Surgery for Reasons Other Than Cosmetic?

There are so many changes that can be made through rhinoplasty surgery.  Plastic surgery specialists, can create a new nose from other parts of the body, such as from the arms and legs. Even people who have lost their nose completely through an accident or other reason can have their nose restored through plastic surgery. Through a pre-operative consultation, the surgery is planned and if necessary, changes in the jawline and cheekbones will also be made.

Is There Any Scarring as a Result of the Rhinoplasty Surgery?

If the rhinoplasty is conducted using the "closed" method, the whole procedure takes place through the nostril and because there are no incisions made outside the nose, there is no visible scarring. 

If the "open" method is used, an incision of about half-centimeter is used in the area called the "columella"  which is found between both nostrils. Due to the nature of the skin found on the nose, the incision and resulting scar is vaguely indistinguishable following the surgery. Additionally, because the incision is placed in the natural fold found between the nose and the lip, it is not directly visible and will not be noticed unless closely examined.

Is It Possible to Improve the Appearance of the Nose Without Surgery?

If the dissatisfaction with the appearance of the nose is minimal, or if surgery is not possible, there is a possibility to use filler to improve the shape of the nose. Of course it is not possible to for the nasal bones or the nose itself to be narrowed or made smaller with filler.  It is possible to turn up the end of the nose with filler, and if there is a hump in the nose, it is possible to reduce or remove the appearance of this curve.

A special evaluation should be carried out by the doctor if the decision is made to carry out this procedure. This procedure should only be carried out if appropriate. The decision to have a cosmetic procedure on the nose without surgery should be made according to the individual's situation, as the results will vary from person to person.  This procedure should not be seen as an alternative to surgery.

The filler to be used should be selected from FDA approved alternatives. This is a very important point in terms of the patient's long term health. At Istanbul Surgical Arts, we are very careful to use only FDA approved products and equipment for all our medical interventions.

What is Hooked Nose Surgery?

If there is no deformity in the bone structure of the nose, the nasal cartilage is smooth in shape, but if the nose tip of nose is not sufficiently raised, a satisfactory result can be obtained by applying a nasal hook. This procedure is actually the process of raising the tip of the nose with the help of threads commonly used by surgeons. The procedure can be easily carried out through the use of local anesthesia. The result achieved is not permanent as the tip of the nose will return to its original position within a few years. If desired the process can be repeated.

As can be understood, this procedure does not make any changes to the bones or cartilages of the nose. For this reason, this procedure will not provide positive results for every patient. You plastic surgeon will be able to tell you whether or not this procedure would produce a positive result for you. If not, your specialist will be able to determine the best path for you.

What is Tip Plasty - surgery for the tip of your nose?

If there is no deformity in the bones (a hump on the back of the nose, the width of the nose is not a problem), attempts to correct only the nasal cartilage is called tip plasty. In general, this surgery is performed in the closed method through the nostrils with the aim to shape the nasal cartilage and, if necessary, remove the tip of the nose. Since there is no interference with the nasal bones in the operation, minimal swelling occurs and only a bandage at the tip of the nose is required for the first few days following the surgery. It can be performed under local anesthesia and sedation, and you can return to normal life on the same day.

As with any cosmetic or medical intervention, your plastic surgeon is the best person to determine whether or not tip plasty is suitable for you by conducting a detailed examination and evaluation. The tip plasty procedure is not appropriate for everyone
